Vulnerable Adults Support Officer with experience preparing care plans for disadvantaged and vulnerable adults is required by a Housing organisation based in Poole, Dorset, South West England.
SALARY: £20,661 - £24,174 (pro rata) + Benefits (Pension Scheme, Free Parking, Flexi-Time)

We have a fantastic new job opportunity for a dynamic and committed Vulnerable Adults Support Officer to join the organisation’s dedicated professional team.
Working as the Vulnerable Adults Support Officer, you will help them to deliver excellent housing related support to empower disadvantaged and vulnerable people within the community to maximum levels of independence through a wide range of activities including, prevention of homelessness, tenancy sustainment, improving health and wellbeing and advocacy, as required.
As the Vulnerable Adults Support Officer, you should have experience of providing support to vulnerable individuals and of preparing and reviewing care plans along with the ability to empathise with, and motivate your clients. Knowledge of the Supporting People Framework and statutory and voluntary sector support services for vulnerable individuals is required. With a flexible and customer-focused working style, you will be calm under pressure and well organised, with the ability to prioritise your workload effectively. Although part of a friendly and supportive team, you will be equally happy using your own initiative and working alone.
This post is subject to an enhanced Disclosure and Barring Service check (DBS), which the organisation will arrange and, as frequent travel to meet clients is necessary, you should have the use of a vehicle for work.

Your duties and responsibilities as the Vulnerable Adults Support Officer:
- Carry out joint needs and risk assessments with the client
- Develop an individual support plan with each client from the needs and risks identified on the joint assessment, with inclusion of family, carers and other external organisations such as social services where appropriate, to inform the way support is provided
- Provide support to residents with a range of vulnerabilities including learning disabilities, mental health issues, physical disabilities, substance misuse issues, behavioural problems, dementia and memory loss. To provide support for victims and perpetrators of antisocial behaviour. Support may include:
- Assistance to understand the tenancy agreement
- Assistance in maintaining independence and control over their lives
- Money management, maximising income and uptake of benefits
- Positively engaging with and supporting victims and perpetrators of antisocial behaviour and tenancy breaches e.g. property condition
- Signposting to other specialist services as needed such as debt advice services.
- Accessing opportunities locally, around learning, employment, leisure and cultural needs
- Early intervention to prevent crisis and deterioration, improve health and general wellbeing including nutrition
- Making a safe informed and independent choice on purchasing services, equipment and adaptations to meet their needs
- Carry out the assessment of referrals and contribute towards prioritising and maintaining any waiting list
IDEAL CANDIDATE REQUIREMENTS
- Experience of providing support to vulnerable individuals
- Experience of preparing and reviewing support/care plans
- Experience of using Microsoft Word and Excel, and mainframe computer systems
- Experience of partnership working
- Good standard of general education, including English and Maths
- Relevant professional qualification or prepared to undertake (CIH, CSHS or similar)
- Strong communication skills
- Good Standard of Literacy
- Enthusiastic and energetic team player
- Ability to empathise with clients
- Ability to communicate with people from diverse backgrounds and at all levels
- Able to maintain professional boundaries and work confidentially
- Ability to use initiative and work without close supervision to tight deadlines
- Ability to prioritise work and time-manage
- Knowledge of Housing Support
- Knowledge of statutory and voluntary sector support services for vulnerable individuals
- Knowledge of social housing and residents’ rights and responsibilities
- Knowledge of welfare and benefit rights
- A driving licence and access to a vehicle for work is required
- Able to visit clients in the locality in their own homes and at times to suit the client, which may involve climbing stairs
- Enhanced CRB / DBS clearance required (Organisation to arrange)
